Registration Forms:

Save Time By Pre-Registering!

We no longer use pre-printed forms
Save time by pre-registering online at www.eayso.org and then bringing 3 printed copies of the registration form.
Computers will be available at the Cuesta College sessions so that you can complete the pre-registration forms. Access to the internet is also available at the Paso Robles Public Library and through many local schools.

Remember To Bring:

Child(ren) – returning 2007 players need not be present
Legal copy of birth certificate – not needed for 2007 players
Registration Fees - vary by date of registration (above)
Three copies of pre-registration form - must have 3
Volunteer forms – every family is asked to provide at least one volunteer age 14+

Player Assessment

Children who did not play in the 2007 season must be rated for skills (even if your child played prior to 2007). Please bring your child in appropriate attire to do a bit of running and ball handling. Soccer shoes are not required.

PARENTS AGREEMENT:

I agree to volunteer in some capacity for my child to be placed on a team.
I understand that request for specific coaches or teammates will not be accepted.
Only the head coach is guaranteed of having their child (s) on their own team.
I understand that the head coach set the days, times and location of practices, and that my child is not guaranteed a certain time or place. I will not be notified of the practice schedule until the season begins in Late August. (Don't panic). Notification can occur as late as one week prior to the first game, in September.
I will arrange transportation to and from practices and games in a timely manner.
I will notify my coach if my child cannot make a practice or a game as soon as possible.
I understand that no transfer or switching of teams shall be permitted.
I understand that my child is placed in a division according to national regulations which is based on his/her birth date. Moving up a division is at the discretion of the Regional board.
